Community Stabilization services are short-term and designed to support an individual and their natural support system following contact with an initial crisis response service or as a diversion to higher level of care.
Assertive Community Treatment
Assertive Community Treatment is a community-based mental health care for individuals experiencing serious mental illness that interferes with their ability to live in the community, attend appointments with professionals in clinics and hospitals, and manage mental health symptoms.
Crisis Stabilization
Crisis stabilization is focused on individuals ages 5 or older who are experiencing an acute crisis of a psychiatric nature that may jeopardize their current living situation. The goals of this service are to prevent hospitalization and establish community stabilization, safety plans, etc.
Services are authorized for up to a 15-day period per crisis episode following a face-to-face intake and may be provided up to 15 consecutive days in each episode or up to 60 days annually.
